Chronic Care management is a program offered by Medicare for patients with two or more chronic health conditions to help improve quality of life and quality of care. This program allows the care coordinator to work with your doctors and care team to set goals and manage your health conditions to avoid unwanted hospital or emergency room visits. The Preventative Care Program will discuss the importance of preventative screening and discuss how our community can lower their risk for higher level health complications
(cancer, hypertension, etc). |
